File a Complaint

If you experienced police misconduct or witnessed police misconduct against another person in Baltimore City, please complete this form to file a formal complaint with the Police Accountability Board.

Please be advised that all information provided will be forwarded to the internal affairs department for the law enforcement agency to conduct an investigation into the misconduct allegation.

You may file a complaint anonymously.
Individuals are not required to provide their name or contact information to submit a complaint. Anonymous complaints will be reviewed and processed to the fullest extent possible based on the information provided.

Your information will be handled with care and confidentiality.
The Baltimore City Office of Equity and Civil Rights - Police Accountability Division makes every effort to protect the privacy of complainants and witnesses. Information shared during the complaint process is used only for investigative and administrative purposes and is disclosed only as required by law or as necessary to conduct a fair and thorough review.
