Five-Year Consolidated Plan

The Consolidated Plan is a document required by the federal government. It helps guide how Baltimore City plans and carries out certain community development efforts. The plan also serves as the City’s official application for funding through four major federal grant programs:

  • Community Development Block Grant (CDBG)
  • HOME Investment Partnerships Program (HOME)
  • Housing Opportunities for People with AIDS (HOPWA)
  • Emergency Solutions Grant (ESG)

A HUD-approved Consolidated Plan is also required for the City to apply for more than 17 additional housing and social service grants from the U.S. Department of Housing and Urban Development (HUD).

Annual Action Plans

The Annual Action Plan serves as the application to the U.S. Department of Housing and Urban Development (HUD) for funding received through four federal formula grant programs:

· Community Development Block Grant (CDBG)
· HOME Investment Partnerships (HOME)
· Emergency Solutions Grant (ESG)’ and
· Housing Opportunities for Person With AIDS (HOPWA)

The Annual Action Plans identify activities that will be undertaken to implement strategies included in the active Consolidated Plan.

CAPERS

Consolidated Annual Performance and Evaluation Report (CAPER) 

The City of Baltimore annually releases a Consolidated Annual Performance and Evaluation Report (CAPER) for the federally funded Emergency Solutions Grants (ESG), Housing Opportunities for Persons With AIDS (HOPWA), and HOME Investment Partnership (HOME), and Community Development Block Grant (CDBG) programs. The report identifies financial resources received through the ESG, HOPWA, HOME, and CDBG programs; describes activities funded through these programs and their relationship to community development objectives; and assesses the City’s success in meeting goals contained in the most recent Consolidated Plan.

PUBLIC COMMENT PERIOD FOR FY 2025 CAPER

The City of Baltimore will release on November 11, 2025, a draft Consolidated Annual Performance and Evaluation Report (CAPER) for the federally funded Emergency Solutions Grants (ESG), Housing Opportunities for Persons With AIDS (HOPWA), HOME Investment Partnership (HOME) and Community Development Block Grant (CDBG) programs. The draft CAPER covers City Fiscal Year 2025 activities – July 1, 2024, through June 30, 2025. The report identifies financial resources received through the ESG, HOPWA, HOME and CDBG programs; describes activities funded through these programs; and assesses the City’s success in meeting goals contained in the Consolidated Plan covering the July 2024 through June 2025 period. Following the public comment period, the CAPER will be submitted to the U.S. Department of Housing and Urban Development (HUD) on or prior to November 28, 2025.
