The Chief of the Fire Department is responsible for maintaining official records for the Baltimore City Fire Department (BCFD) under the Maryland Public Information Act.

How to Request Records
Request EMS Record:
To request an EMS report from the Baltimore City Fire Department (BCFD), you must provide the following:
- A written request
- A completed and signed HIPAA authorization form (signed by the patient or an authorized representative)
Download HIPAA Authorization Form (PDF) - A $6.50 check made payable to the Director of Finance
No fee is required if you are requesting your own records.
