Request an EMS Report

To request an EMS report from the Baltimore City Fire Department (BCFD), you must provide the following:

Required Documents

  1.  A written request
  2. A completed and signed HIPAA authorization form (signed by the patient or an authorized representative)
    Download HIPAA Authorization Form (PDF)
  3. A $6.50 check made payable to the Director of Finance
    No fee is required if you are requesting your own records.

Alternative Options

If you cannot obtain a signed HIPAA authorization form, you may serve BCFD with a subpoena that complies with the Maryland Confidentiality of Medical Records Act (§ 4-301 et seq., Health General Article, Maryland Code).
