The Baltimore City Continuum of Care (CoC) is a U.S. Department of Housing and Urban Development (HUD) initiative that includes organizations that serve people experiencing homelessness, persons with lived experiences of homelessness, and other stakeholders with a passion for ending homelessness. The CoC carries out responsibilities assigned by HUD's CoC Program and coordinates available resources and stakeholders' efforts to make homelessness rare, brief, and non-recurring in Baltimore City.

CoC Leadership

Lead Agency

The Mayor's Office of Homeless Services (MOHS) is the designated lead agency for the CoC. MOHS is responsible for implementing Federal, State, and local policy; promoting best practices, and administering and monitoring homeless services grants. MOHS provides approximately $48 million annually for homeless diversion programs.

As the Collaborative Applicant and Lead Agency for the CoC, MOHS fulfills the following roles:

  • Submit the annual CoC Consolidated Funding Application
  • Apply for a grant for Continuum of Care planning.
  • Lead the Continuum’s Homeless Management Information System (HMIS).
  • Coordinate, direct, and advocate for the work of the Continuum and the Board.
